Pre-Approval and Trial Day Request

Never been to our location or not quite ready to commit? No problem.

We offer a one-time pre-approval and trial day so you can test out our location. We just ask for the basics to get you started. Proper licensing and permits are still required, but for your trial day, we only ask for proof of insurance, no need to add us as an additional insured just yet.

This way, you can try it out without the extra work with your insurance in case it’s not the right fit for you.

Please have the following documents ready before you apply:

  • Your Menu

  • Buckeye Business License

  • Maricopa County Health Permit

  • Certificate of Insurance

Vendor Application

Already pre-approved or ready to commit without a trial? Great! You can fill out our official vendor application to get started. This time, we’ll need a bit more information from you.

Once your application is processed and approved, you’ll be asked to sign our vendor agreement and pay the annual vendor fee. After that, you’ll be all set to get on the schedule!
