Vendor Application

Read through all our FAQs and ready to apply? Great! You can fill out our official vendor application to get started.

Once your application is processed and approved, you’ll be asked to sign our vendor agreement and pay the annual vendor fee. After that, you’ll be all set to get on the schedule!

Please have the following documents ready before you apply

  • Your Menu

  • Buckeye Business License

  • Maricopa County Health Permit

  • Certificate of Insurance (additional insured must be added this time)

  • Your Logo

  • Up to 6 photos you want to feature
